The author's name is not on the item, but the poem is attributed to Thomas Campbell by T.L. Philbrick in "British authorship of ballads in the Isaiah Thomas collection," v. 9 of "Studies in bibliography, papers of the Bibliographical Society of the University of Virginia," p. 255-258.
